According to the [[U.S. Census Bureau]], Middletown has a total area of {{convert|0.32|sqmi|sqkm|2}}, all land.<ref name="CenPopGazetteer2023"/> Coon Creek, an east-flowing tributary of the West Fork of the [[Cuivre River]], crosses the southwest corner of the city. The Cuivre River is a direct tributary of the [[Mississippi River|Mississippi]].

The [[population density]] was {{convert|521.9|PD/sqmi|PD/km2|1}}. There were 104 housing units at an average density of {{convert|325.0|/sqmi|/km2|1}}. The racial makeup of the city was 93.4% [[White (U.S. Census)|White]], 0.6% [[Native American (U.S. Census)|Native American]], and 6.0% from two or more races.

There were 79 households, of which 24.1% had children under the age of 18 living with them, 41.8% were [[Marriage|married couples]] living together, 8.9% had a female householder with no husband present, 6.3% had a male householder with no wife present, and 43.0% were non-families. 39.2% of all households were made up of individuals, and 19% had someone living alone who was 65 years of age or older. The average household size was 2.11 and the average family size was 2.80.

[[Hispanic (U.S. Census)|Hispanic]] or [[Latino (U.S. Census)|Latino]] of any race were 0.50% of the population.

There were 90 households, out of which 23.3% had children under the age of 18 living with them, 50.0% were [[Marriage|married couples]] living together, 11.1% had a female householder with no husband present, and 36.7% were non-families. 32.2% of all households were made up of individuals, and 65.7% had someone living alone who was 65 years of age or older. The average household size was 2.21 and the average family size was 2.79.
