On March 6, 2007, the United States [[Chemical Safety Board]] (CSB) released its final report on the incident indicating that an operator bypassed a safety interlock and drained a pressurized reactor instead of an empty one they were in the process of cleaning. The report listed as root causes, that Formosa-IL and Borden Chemical "did not adequately address the potential for human error.<ref>{{cite journal |last1=Long |first1=Lisa A. |last2=Lay |first2=James |last3=Leskin |first3=Katherine |last4=McClure |first4=Randy |last5=Smith |first5=Allen |title=Vinyl chloride monomer explosion |journal=Process Safety Progress |date=March 2008 |volume=27 |issue=1 |pages=72β79 |doi=10.1002/prs.10233|s2cid=110009677 }}</ref><ref>{{cite web |title=VINYL CHLORIDE MONOMER EXPLOSION |url=https://www.csb.gov/assets/1/20/formosa_il_report.pdf?13838 |publisher=U.S. CHEMICAL SAFETY AND HAZARD INVESTIGATION BOARD |access-date=February 20, 2023 |quote=REPORT NO. 2004-10-I-IL}}</ref>
