A downstream temperature indicator, the sensor for which was located in the tail pipe between the pilot-operated relief valve and the pressurizer relief tank, could have hinted at a stuck valve had operators noticed its higher-than-normal reading. It was not, however, part of the "safety grade" suite of indicators designed to be used after an incident, and personnel had not been trained to use it.
