Millersburg is the southwestern terminus of [[U.S. Route 209]], which leads northeast {{convert|44|mi}} to [[Pottsville, Pennsylvania|Pottsville]] and {{convert|211|mi}} to its northeastern terminus north of [[Kingston, New York]]. [[Pennsylvania Route 147]] passes through the center of Millersburg as Market Street, leading north (upriver) {{convert|29|mi}} to [[Sunbury, Pennsylvania|Sunbury]] and south (downriver) {{convert|13|mi}} to [[U.S. Route 22 in Pennsylvania|U.S. Route 22]] near [[Duncannon, Pennsylvania|Duncannon]]. The [[Millersburg Ferry]] leads west across the Susquehanna to [[Buffalo Township, Perry County, Pennsylvania|Buffalo Township]] in [[Perry County, Pennsylvania|Perry County]], allowing a connection to U.S. Routes [[U.S. Route 11 in Pennsylvania|11]] and [[U.S. Route 15 in Pennsylvania|15]].
